Certificate Graduation Request Form

There is no additional graduation audit fee for certificates only if you have completed the Online Request for Graduation, for one of the degrees listed on your record, and made the graduation audit fee payment. Additionally, both the certificate and the degree must be complete and certified for the same graduation date.
