Certificate Graduation Request Form

PLEASE READ before completing this form:

If you have completed the Online Request for Graduation for one of the degrees listed on your record and made the graduation audit fee payment, please complete the Certificate Graduation Request at https://forms.tesu.edu/certgradreqn.php.There is no fee for this addition as long as both the certificate and the degree are complete and certified for the same graduation date and payment was made. By selecting this option you are certifying that the above statement is true.

If you are not graduating with a degree at this time, you are required to complete the form below and submit the certificate graduation audit request fee. If you are applying for multiple certificates in this graduation cycle, please complete the below form, for the first certificate, and access https://forms.tesu.edu/certgradreqn.php to apply for all other certificates at no additional fee.

Important note: If you are not a currently enrolled student, you must go through the Inactive Student graduation process first: https://www.tesu.edu/current-students/inactive-student

Credit and debit card payments are handled through a third party payment processor. A non-refundable convenience fee of 2.95 percent (with a minimum charge of $3.00) will be added to your total charges for this processor. The University does not receive any portion of convenience fee payments.

To avoid convenience fees you can pay via electronic check/ACH payment. Electronic check/ACH payments require your bank routing and account numbers. The routing number and checking account number can be found on the bottom of your personal check.
